About the CT ACHIEVE Initiative

The Connecticut ACHIEVE Initiative is funded through a cooperative agreement between the Connecticut Cancer Partnership and the Department of Public Health.  The initial grant period is from 2011 through 2013 and was established to assist 4 Connecticut communities in their efforts to convene a Community Health Action Response Team (CHART) at the local level to reduce the incidence of and risk factors for cancer and chronic disease through policy, systems, and environmental change.  The funding provides seed money and training opportunities for these communities as they establish a framework for change to increase opportunities for physical activity and access to healthy food.

The 2011 communities are:

  • Bridgeport (managed by The Witness Project)
  • Wethersfield (managed by the Central Connecticut Health District)
  • Southbury (managed by the Paumpaurag District Department of Health)
  • New Britain (managed by the Community Health Center)

    The Eastern Highlands Health District was funded to provide technical assistance and training to these four communities, with assistance by Ledge Light Health District
