Building Healthy Communities
Working with county-based coalitions to address policy, systems, and environmental changes to improve health and reduce chronic disease rates, a specific focus of this work is on elimination of health disparaties. The 3 main Strategic Directions for this CDC-funded initiative are: healthy eating & active living; tobacco-free living; and quality clinical preventive services.

The Center for Disease Control and Prevention awarded 61 grants in the fall 2011 to counties and states across the nation to reduce the burden of chronic disease and eliminate health disparities through policy, systems, and environmental changes to increase opportunities for healthy living. The State of Connecticut was awarded one of these grants to work with the counties in the state with under 500,000 population, in a Capacity Building phase to prepare for an application for implementation funding in 2013 and 2014. The counties funded through this initiative are: Litchfield ~ Tolland ~ Middlesex ~ Windham ~ New London
