Sodium Chloride damage reporting form
PA 23-21, Sec. 19 requires Health Districts:
- to establish an electronic reporting system for homeowners to report to the health district any damage as a direct result of sodium chloride
- to submit reports annually to the Office of Policy Management (OPM) annually
Homeowners may use this Reporting Form to electronically submit information to Eastern Highlands Health District. IMPORTANT: If submitting photos, please submit them in the smallest resolution (file size) possible or email them directly to EHHD@EHHD.ORG.
